Audit trail
Every change (assessments, refinements, runs, reviews, sign-offs, envelopes, ADM entries, vendor reviews, settings) is appended to a hash chain. Altering, deleting or reordering any past event breaks the chain from that point.
Tamper-evident, not tamper-proof: someone with database access could rebuild the whole chain. Record the chain head elsewhere (it is printed in each board pack) to detect that.
